The physical exam you received as required by the University and Health Services will be accepted by the Sports Medicine Department. YOU DO NOT HAVE TO RECEIVE A SEPARATE PHYSICAL EXAMINATION TO PARTICIPATE IN ATHLETICS.
Please use the checklist on the first page of the questionnaire to make sure that you have all of your medical forms completed. The following forms must be on file in the Sports Medicine Department PRIOR to any team activity or you will not be allowed to participate:

First-Year Athlete Medical History Questionnaire
    • Please note: All student-athletes must sign consent form and provide a copy of insurance card (front & back) in the medical history form.

ImPACT Test
  • All First Year and Transfer Student-Athletes will take a baseline ImPACT test prior to their season. Print the confirmation sheet given at the end of the test, and attach to the medical questionnaire packet. To access the code needed to begin the test click on
