Please use the checklist on the first page of the questionnaire to make sure that you have all of your medical forms are completed. Returning Student-Athletes must have on file in the Sports Medicine Department the following forms to participate (Click to Download):

Pre-participation Physical Evaluation Form (PDF) -
your health care provider/doctor must complete. VERY IMPORTANT: If your healthcare provider has its own pre-participation physical or medical forms, we will accept, as long as they STATE YOU ARE CLEARED FOR PHYSICAL ACTIVITY.
2.
Medical Questionnaire for Returning Athletes (PDF) - please include copy of insurance card (front & back).
These forms must be on file in the Sports Medicine Department PRIOR to any team activity or you will not be allowed to participate.
Do not delay turning in your pre-participation Physical Evaluation Form completed THIS SUMMER through your health care provider. If you are unable to schedule a physical to complete the pre-participation Physical Evaluation Form through your health care provider,
DREW UNIVERSITY HEALTH SERVICES is an option. There will be a fee for the student-athletes not enrolled in the student health insurance plan. If you have the student health insurance plan, there is a charge, but it is not out-of-pocket.
Please set up an appointment to complete pre-participation Physical Evaluation Form as soon as possible through health services as there are limited appointments available.
